CONSENT TO PROCESSING OF PERSONAL DATA 


in accordance with Art. 6 par. 1 letter a) and Art. 7 of the Regulation of the European Parliament and of the Council (EU) 2016/679 from 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, which repeals Directive 95/46/EC (General Data Protection Regulation)


I grant to the Operator: SOS electronic s.r.o., with registered office at Pri prachárni 16, 040 11 Košice, ID number: 31703186, registered in the Commercial Register of the Municipal Court of Košice, section: Sro, entry no. 5791/V (hereinafter referred to as the "Operator")
 

the consent to the processing of my personal data

in the form of obtaining, collecting, making available, cross-border transfer of e-mail contacts 

for the purpose of sending newsletters.


This consent to the processing of personal data is a freely given, specific, informed, and unambiguous expression of my will for the time necessary for processing for the given purpose. I was informed that after the expiration of the period for which I granted this consent, or after its cancellation, my personal data will be mechanically destroyed (shredded) and will not be further processed in any database.


Before giving this consent, I was informed about the following facts:

 

  1. my personal data may be provided to other recipients, an authorized employees of SOS electronic s.r.o. and business partners of the company 
  2. my personal data, the provision of which is voluntary, will be kept during the period of validity of the consent and will not be processed for any purpose other than the one for which it was obtained. 
  3. as the person concerned, I have the right to request the access to my personal data, I have the right to their correction or deletion or restriction of processing, the right to object to processing, as well as the right to data transfer.
  4. in a justified case, as the person concerned, I have the right to file a complaint with the supervisory authority, which is the Personal Data Protection Office of the Slovak Republic.
  5. I acknowledge that the cross-border transfer of my personal data to a third country does not take place. 
  6. I acknowledge that there is no automated decision-making, including profiling. 

 

I have the right to withdraw my consent at any time, even before the expiration of the period for which this consent was granted. I can exercise this right by e-mail, by phone or by sending a written request by registered mail with the text "GDPR" to the contact details below. 

Withdrawal of consent does not affect the lawfulness of processing based on consent prior to its withdrawal.
